Fellows nearing the end of their tenure and their host researchers are asked to fill out this questionnaire, which will be used to measure the current performance of services provided under the program, as well as to improve the fellowship program.

Your personal input will be used exclusively as criteria for improving the fellowship program, and will not be released to the public.  However, the data compiled through this survey may be released for the purpose of enhancing the program.

The questionnaire is divided into two parts, one for the fellow and the other for the host to fill out. Please choose one and answer the questions.

The questionnaires should be returned via facsimile or by post until the specified deadline.  Your deadline will be informed by mail separately according to the termination month of your fellowship; 4 times per year  —  late June, late September, late December, and late March.
