逆転写酵素阻害剤とプロテアーゼ阻害剤の両者がAIDSの治療に用いられるようになってHIV感染症の臨床像はかつてないほど大きく変化した。まさにAIDSとHIV感染症は「死の病」から「治療可能な慢性感染性疾患」へと変貌したと言ってよい。しかしどの病期でどの組み合わせの治療法をとるべきかについての指針は必ずしも確立されていない。しかも、HIV-1は両阻害剤に対して容易に耐性を発現し、その耐性は多剤耐性であることが多い。事実、いかなるプロテアーゼ阻害剤を含んだ多剤併用療法にも反応しない症例が多数存在する。本研究では、抗ウイルス治療を受けているHIV-1感染症例のHIV-1株を単離、その特性につて解析すると共に、HIV-1が耐性を発現しにくい薬剤、発現しても他薬剤との交差耐性を有しない新規の逆転写酵素阻害剤、プロテアーゼ阻害剤、更にインテグレース阻害剤などの開発をX線結晶解析などを用いたデザインを通じて進め、臨床応用の道を開く。更に新しいクラスの抗ウイルス剤の開発を進めるとともに作用機序の異なる免疫療法や遺伝子治療などと抗ウイルス剤多剤併用療法を組み合わせた新しい治療法についての研究を展開する。

1. 抗ウイルス薬の投与を長期に亘って受けている本邦のHIV感染症例から臨床HIV株を分離、耐性変異出現の態様について解析を進める。さらに同定した耐性付与遺伝子変異をクローン化し、その特異性についてウイルス学的、酵素学的に検討、これらに感受性を有する新規の抗ウイルス剤を同定する。 2. 新規の逆転写酵素阻害剤 (RTIs) の開発を行う。これらについて耐性誘導を行うと共に種々の薬剤変異株に対する活性の検討を行う。このようにして得られた有望な化合物については新規のプロテアーゼ阻害剤との併用も含めて臨床試験を目指す。 3. 新規のプロテアーゼ阻害剤(PIs)の開発を行う。さらに選別した化合物について既存のプロテアーゼ阻害剤との併用での抗ウイルス活性の検討、耐性発現のスペクトラムin vitroでの解析（耐性誘導）などを行う。得られた遺伝子変異を有した感染性クローンを用いた検討や変異プロテアーゼのX線結晶構造解析を行う。 4. 新たな作用機序を持つ抗HIV剤（インテグレース阻害剤や接着阻害剤など）の基礎的検討を始める。さらにより活性の高い化合物の開発、デザインを進め、得られた化合物について耐性機序の検討などを行い、前臨床試験、臨床応用を目指す。 5. HIVに対する免疫応答の役割についての基礎研究を行い、HIVに対する細胞性・液性免疫応答の誘導法の開発（免疫療法）を目指す。さらに免疫療法・遺伝子治療と多剤併用療法の組み合わせなどについても検討する。

４−１　研究計画、目的に対する成果

本研究では既存のRTIs, PIsに対して活性を有する新規のRTI, PIの開発及びその活性発揮のメカニズムの解析を行い、複数の成果が得られた。 RTIsに関して、複数のRTIsに対して高い薬剤耐性を有する変異株、HIVMDRの発現機序の解明を行った。HIVMDRの発現にはRT領域の５個のアミノ酸の置換が必要とされているが、本研究においてはこれらのアミノ酸変異がウイルスの増殖能(fittness)に与える影響や5種類のアミノ酸変異で最初に出現するGIn151Metの出現のメカニズムを種々の組み換え感染性HIVクローンと組み換え逆転写酵素を作成して検討した。 既存のRTIsと交差耐性を示さない新規のRTIsの開発として、acyclic nucleoside 誘導体や、3'位にOH基を有しながら高い抗HIV活性を発揮する一連の4'-修飾ヌクレオシド誘導体（4'-ethynyl-2'-deoxynucleosides: 4'-E-dN） を同定、そうした抗ウイルス活性物質のいずれもがヌクレオシド系のRTIsとしてHIVのproviral DNA chain terminationを起こすことで抗ウイルス活性を発揮していることを示した。とりわけ、一連の4'-E-dN誘導体は既認可のRTIsに比較して極めて高い抗ウイルス活性を発揮、しかも全ての既認可のRTIsに対して高い薬剤耐性を示す臨床HIV変異株の増殖も野生株と同様に抑止することを示した。 さらに、薬剤耐性を誘導しにくい/しない新規のPIsの研究・開発に取り組み、柔軟な構造のP2' 部分を有することで他種類のPIsに対して高い交差耐性を有する変異株を含む広いスペトラムのHIVに強い抗ウイルス活性を発揮するJE2147/AG1776、およびプロテアーゼの活性部位のアミノ酸の側鎖ではなく主鎖に結合することで耐性が発現しにくく，しかもHIVMDRなどにも極めて高い活性を発揮するUIC94003/TMC126などの新世代のプロテアーゼ阻害剤を同定した。更にUIC94003/TMC126の誘導体で耐性発現やプロテアーゼとの結合態様の異なる複数のリード化合物を見い出した。 新たな作用機序の抗HIV剤の開発について、インテグラーゼ阻害剤の研究は研究スタッフ（コアメンバーの原田教授のグループ）の他大学への移動に伴い進展が得られなかった。また免疫療法・遺伝子治療と多剤併用療法の組み合わせの検討についても進展が得られなかった。しかし、新規のクラスの抗ウイルス剤として下記に示すように一連の接着阻害剤（CCR5阻害剤）を同定、解析を進めた。

４−２　研究計画、目的外の成果

新たな作用機序を有する抗HIV剤の開発の１環として、1999年より新規のCCR5 antagonistsの一員であるspirodiketopiperazine (SDP) 誘導体の研究に着手、プロトタイプの化合物 (E913) を同定、その抗HIV活性と作用機序を明らかにした。E913 とその誘導体は現在本邦で用いられている全てのRTIs及びPIsに対して高度の薬剤耐性を有する臨床R5-HIV変異株に対して極めて強い抗ウイルス活性を発揮、CXCR4阻害剤と併用すると何れの臨床株についてもほぼ完全にそれらの試験管内での増殖を抑止することを示した。さらにSDP誘導体のCCR5への結合様式の検討、ケモカインへの阻害の影響などについての検討も行った。 そして E913 よりも強力かつ薬物動態に優れた化合物の開発に成功、現在 in vivo (SCID-huマウスAIDSモデル)での抗ウイルス活性の解析を進めている。

４−３　研究成果の展望

満屋は米国国立衛生研究所における20年に及ぶ研究生活から得られたAIDSの治療法の開発に対する経験を元に新規の化合物として4'-E-dN、JE2147/AG1776、UIC94003/TMC126、更にE913とその誘導体を同定、それらのウイルス・酵素・薬理又は結晶解析学的特性を明らかにした。 学術的な意義としてRTIs, PIIsについては薬剤耐性HIVに有効な新規薬剤の開発が一刻も早く望まれる中で有望な複数の化合物を発表し、しかもこうした新規の物質がHIVの薬剤耐性獲得に抵抗するメカニズムについて遺伝子、酵素さらにX線結晶構造学な検討を加え、それらは今後のより新たな薬剤の開発のための有用な知見となっているのは特筆してよいと思われる。 CCR5阻害剤については、既に数種類の抗HIV剤が報告されているものの、いずれも臨床応用には至っていない。本研究では新規のCCR5阻害剤、spirodiketopiperazine(SDP)誘導体を同定、その後、更に強力で臨床応用の可能性のあるリード化合物を多数見い出した。 今後は最も臨床応用に適した化合物を多面的な検討を通じて選択、SCID-huマウスAIDSモデルなどを用いて、in vivo での抗ウイルス活性の解析など前臨床開発に資する基礎実験を進める。それとともに、標的細胞膜上に表現されているCCR5との結合様式など、その抗HIV作用の機序についての基礎的検討を行う。このような検討の中で強力な抗HIV作用を維持しながらケモカインとCCR5との結合は軽度しか阻害しないCCR5阻害剤を見い出しており、これらの知見は今後生体の分子をターゲットとする新規抗HIV剤の開発の際の興味ある情報となっている。 　4'-E-dN、JE2147/AG1776、UIC94003/TMC126、E913 とその誘導体、及びそれらのリード化合物の何れもが新規の化合物として特許の申請が終了又は予定されており、さらに活性、動態などの改善を加え、臨床応用への可能性が示されている。

４−４　本事業の趣旨に鑑み、果たした役割

耐性HIVに対する新規抗HIV剤の開発が急務の中で、既存のクラス(RTIs, PIs)と新規のクラス(CCR5阻害剤など)の抗HIV剤の研究・開発は焦眉の課題である。 本研究では上述の通り、多数のRTIs, PIs, CCR5阻害剤の開発に成功、さらにそのうちの幾つかが前臨床・臨床試験の準備段階にある。これらの研究・開発を進めることができれば、日本からの初の抗HIV 剤の開発が実現する可能性がある。 また、本研究で対象となった化合物はいずれも新規のものであり、これらをプローブとしてウイルスの酵素の諸特性の解析、またケモカインレセプター、CD4分子、HIVの相互反応などの基礎研究を今後とも継続して行うことにより、基礎と臨床の両分野での「HIV学」の進展が期待される。
